2/18/2024 EnchantaVaughn Approved Behavior: As QMHP came to visit the home, the client was witnessed interacting with her family in a helpful and positive way. Client participated in family exercise in a cooperative manner, reviewing rules and expectations for new team building skills. Client was given positive feedback and engaged in one on one. Client was seen conversing with her mom and practicing responsibility by encouraging her to do his part of the chores at which her mother reports behaviors last up for 20 minutes in duration. Behavior: Client displays leadership skills in the home setting and is always volunteering to help her mom around the house. During the second portion of the session, client displayed off task behaviors such as her looking at her mother and siblings while the session was being conducted. She was redirected by QMHP to focus back on the session at which she was able to regain focus of the session once redirected at which behaviors lasted up for 20 -30 minutes in duration. Intervention: QMHP discussed and spoke with mom and client about in-home services and how they liked it so far. QMHP assessed and monitored clientâs mood and behavior. QMHP probed in order to process and explore causes of clientâs increase in possible reasons for setbacks and failures. QMHP redirected client a few times in the session to refocus and pay attention in order to get most out of it. QMHP educated client on ways to prevent distractions from affecting her in the home. Intervention: QMHP reviewed and discussed objectives and goals that will allow and teach client how to channel her reported behaviors within the home and community environments. QMHP explained the importance of removing distractions when trying to complete an assignment or simple task. QMHP encouraged client to focus on one task at a time and then she can engage in activities and other hobbies of her choice, as she see fit. Response: Client administered positive behavioral expectations, reducing ADHD in the home. Client anger has been very intense this week. Client exhibited techniques for appropriate self-expression and active listening so that she could maintain a balanced mental state. Client understands that problem solving starts with thinking before acting. Client focused on things she is good at and loves to do at home in order to stay motivated and make something out of herself. Response: Client utilized conflict management skills to reduce tense emotions when faced with challenges in the home. Client informed counselor that most of her behaviors stem from past family history and assuming the worst. Client communicated that she wants to work independently instead of as a family so that she could gain clarity of her personal feelings without taking it out on someone she loves.
